After thinking some more, I fell that there is no clear cut answer. If at least one of the calls is an on-net call, then one can argue like I did, Vonage is not doing anything additional than the basic service. But if both the calls are to PSTN, then they are incurring the charges for two calls simultaneously, a deviation from the norm.
